Community Partnerships
Partnerships are the foundation of CHAS. We work in sustained, accountable collaboration with community organizations, health systems, and civic institutions. Our partnerships are designed to build community power, not institutional reputation.
CHAS partners with one of the nation's largest safety-net health systems to redesign how community-based interventions are identified, tested, and integrated into care delivery. This partnership, which includes Cook County Health and the Cook County Department of Public Health, creates a direct pathway from community innovation to system-wide adoption, with a combined institutional co-investment and a shared commitment to sustainable public financing.
In West Garfield Park, CHAS collaborates with the Maafa Redemption Project and the Sankofa Wellness Village, a $56 million community-owned development anchored by the newly opened Sankofa Wellness Center, which brings together primary care, behavioral health, the YMCA, and workforce development services for the West Side.
This partnership identifies and supports community organizations to participate in the grants program, reaching networks that institutional partners cannot access alone, and ensures that research findings route back to community residents, policymakers, and other safety-net systems, considering replication
